Requests were sent July 22, 2026 to Waterbury Police, Waterbury Fire/EMS, and the Connecticut Office of the Chief Medical Examiner. Acknowledgments, tracking numbers, invoices, exemptions, no-record explanations, and productions will be logged here.

How missing records are described
The family reasonably expected the responsible agencies to create, retain, or produce enough documentation to reconstruct the death response. When an expected source record is absent from the supplied materials and current archive, it is classified as an authority-side production or recordkeeping shortcoming until the custodian produces it or explains its status in writing.
Reason still unknown
The record may never have been created, may no longer be retained, may be held elsewhere, or may be withheld under a stated legal basis.
Written agency answer required
The classification changes when the agency produces the record or provides a sufficiently specific written explanation.
Evidence boundary: Nonproduction is not, by itself, proof that a record was destroyed, concealed, altered, or deliberately withheld. Those are separate claims requiring supporting evidence.
REQUEST 01 · WATERBURY POLICE
Complete incident file and controlling 2021 policies

The request seeks every report, supplement, handwritten or electronic note, supervisor review, CAD attachment, dispatch and radio record, scene photograph, video, diagram, evidence/property record, body-camera assignment and audit record, retention or deletion record, medical-examiner communication, and production or exemption log.
It also seeks the policies effective November 21, 2021 governing body-worn cameras, sudden or unattended deaths, scene photography, evidence preservation, medical-examiner notification, reporting, record retention, and supervisor review.
Current shortcoming: The supplied record does not contain enough visual, evidentiary, or narrative material to independently reconstruct the police scene response.
REQUEST 02 · WATERBURY FIRE/EMS
Complete medical-response and device record

The request seeks CAD and radio records, complete crew narratives and amendments, CPR documentation, AED and monitor event logs, rhythm strips, interventions and medications, responder identities and assignments, photographs or video, QA and supervisor review, communications with police and OCME, and the policies governing death responses and record retention.
Current shortcoming: The family archive does not contain the complete operational, clinical-device, QA, and communications record needed to compare the emergency response with the 911 call and police narrative.
Privacy note: Medical portions may require proof of relationship, estate authority, or another lawful release process. A request for such documentation will be logged as a processing requirement—not treated as a denial.
REQUEST 03 · CONNECTICUT OCME
Complete medical-examiner case file

The request seeks referral and intake records, call logs, investigator notes, jurisdiction acceptance or declination records, the factual basis and decision-maker for the disposition, communications with police, EMS, clinicians, funeral home, crematory and family, photographs and documents reviewed, body-release and cremation records, policies applied, amendments, metadata, and audit history.
The request also asks OCME to state in writing, for each unproduced category, whether the record was never created, is no longer retained, was destroyed or deleted, was transferred to another custodian, or is withheld under a stated legal basis.
Current shortcoming: The archive does not contain a complete OCME file sufficient to reconstruct notification, jurisdiction review, decision-making, communications, or disposition.

What happens next
Requests sent
All three priority requests were transmitted on July 22, 2026. Internal copies and submission details are preserved in the controlled evidence register.
Acknowledgments and tracking
Receipt confirmations, tracking numbers, medical-record requirements, fee estimates, and assigned custodians will be logged when received.
Production comparison
Every produced record will be matched to the request category and checked for missing attachments, unexplained gaps, duplicate exports, metadata, and stated exemptions.
Evidence-led updates
The tracker will distinguish complete production, partial production, written no-record response, lawful withholding, disputed response, and unresolved silence.
Public conclusion: The absence of expected records is already a documented transparency and reconstruction problem. Responsibility for the underlying cause remains subject to the agencies’ written responses and the controlling policies.
